Semaglutide
In an early study of 2,000 obese adults compared people using semaglutide plus a diet and exercise program with people who made the same lifestyle changes without semaglutide. After 68 weeks, half of the participants using semaglutide lost 15% of their body weight, and nearly a third lost 20%.

Lumen device/total metabolism tracking
The Lumen device measures carb and fat burn using a highly validated test of CO2 that is detected when patient breathes into the device. This has been validated to closely approximate real metabolism. Patients just need to breathe into the device daily to get helpful recommendations and better understand how their body burns fat.

Follow our personalized program to achieve the most Fat loss using safe and effective medications and Program therapies

Schedule of health coach visits for 12-week program (8 visits)
Visit 1
Discussion of body composition and patient goals.
Encourage patient to come to the office for bi-weekly visits if geographically feasible. Discuss role of Lumen device in terms of metabolism tracking.
Action: Advise patient to keep food diary for next week and to use the Lumen device daily.

Visit 2
Discuss food diary and make appropriate suggestions for improvement
Focusing on total caloric intake and healthy eating. Go deep here as much as time allows. Have to set the foundation for healthy eating. Explain that we do not recommend "dieting". Goal is to change our eating to a healthy eating that can be maintained well after program is over.
Action: Ask patient to keep diary of exercise for next week. If possible suggest they get an activity tracker for more accurate tracking.

Visit 3
Discuss exercise pattern from prior week and make appropriate recommendations.
Focus on anaerobic exercise for at least 30 minutes 4-5 times per week. Aerobic should be on top of this but if they only have time for one or the other, then advise anaerobic.
Action: Advise patient to get new body composition prior to next week's visit.

Visit 4
Discuss progress/lack of progress vs week 1 baseline if we have a new body composition.
Keep in mind that they are in the building stage of the medication, so we are not expecting a lot of fat loss at this point. 3-4 lbs of fat loss in the first month would be considered a success. Many patients with 75-100 lbs to lose report losing up to 25 lbs in first month. Ascertain if patient has been compliant with dietary and exercise recommendations and medication adherence.
Action: If there has been no fat loss or even fat gain, then need to discuss the case with medical director for possible additional interventions. Advise patient to track sleep for next week preferably with sleep tracking device but if not, then just manual tracking (time to bed, time waking up, how many times did they awake for the night, sleep hygiene questions.)

Medical Director Visit
Medical Director Visit
Points to consider seeking to uncover reasons for weight loss resistance. Review initial labwork looking for sub-optimal areas that could be impacting weight loss as well as sleep quality.

Action: Medical director to advise what they feel is the biggest problem area(s) that require focused attention.

From areas below, spend the time reinforcing needed behavior in 1-2 of the most problematic areas.
- Diet/Nutrition (appetite suppressant), Exercise, Medication compliance, Sleep, Low IGF-1 (GHRH therapy), GI issues - GI testing, Other metabolic issues

Visit 7

Updated at 2 p.m. Sunday
VIRGINIA — New details on timing and expected snowfall totals have been released as a winter storm moves over Northern Virginia on Sunday.
The National Weather Service issued a winter weather advisory for several counties in the Washington, D.C. region, including Loudoun, Fairfax, Arlington, Falls Church, Alexandria, and Prince William counties in Virginia, along with Manassas and Manassas Park. The warning is in effect Sunday until 7 p.m."Snow will overspread the area Sunday morning and continue through the afternoon before tapering off during the evening," the National Weather Service said. "Snowfall rates could approach 1 inch an hour at times."
As much as 4 inches of snow is expected to fall in Loudoun County, while the D.C. metro will likely see 2-3 inches. Farther south, less than an inch of snow is predicted in Fredericksburg.
Travel conditions are expected to quickly deteriorate on Sunday. Gusty winds could also bring down tree branches, forecasters said.

"Persons should delay all travel if possible," the National Weather Service said. "If travel is absolutely necessary, drive with extreme caution and be prepared for sudden changes in visibility."
Once snow moves out of the region, a polar vortex — a stormy ring that typically keeps the coldest air locked up near the North Pole — will settle over Northern Virginia and bring bitter and dangerously cold temperatures to the Baltimore region.
Temperatures on Sunday will reach the 30s before falling into the mid-20s on Monday. Lows will plunge into the single digits before warming just slightly on Tuesday. According to the NWS, Tuesday will see a high of 25 degrees and a low of 9.
Here's a look at the detailed D.C.-area National Weather Service forecast through Tuesday:
Sunday: Rain and snow likely before 2pm, then snow. The snow could be heavy at times. High near 39. North wind 7 to 11 mph, with gusts as high as 21 mph. Chance of precipitation is 100%. Total daytime snow accumulation of 1 to 3 inches possible.
Sunday Night: Snow likely, mainly before 9pm. The snow could be heavy at times. Cloudy during the early evening, then gradual clearing, with a low around 20. Northwest wind 11 to 15 mph, with gusts as high as 25 mph. Chance of precipitation is 60%. New snow accumulation of less than one inch possible.
M.L.King Day: Mostly sunny, with a high near 25. West wind 13 to 18 mph, with gusts as high as 31 mph.

LEESBURG, Va. — A family in Leesburg, Virginia who does so much for the community lost everything in a fire Sunday. Now their loved ones are asking the community to give back to them.
Janae Wheeler runs the company VVS Events with her husband, Sir. Despite losing their home, they are continuing to show up for the DMV, making sure the holiday events they are a part of go on.
“The show must go on," said Janae. "There are people who are in need and we can’t stop what we’re doing to continue to service people’s needs for joy and kindness at this time just because we’re having a setback.”
A “setback” is a humble way of describing what the Wheeler family went through on Sunday.
Janae said she was enjoying a cup of coffee with her husband after returning from the Brambleton Town Center Winter Wonderland Event they just helped out at, when she and her husband smelled something coming from their basement.
“My husband went to investigate it and he opened the basement door and black smoke came up into the hallway," said Janae.
They immediately got their four kids, their grandmother, and their dog out of their home on Leeland Orchard Road. She said one simple thing is what helped her family get to safety, and now she wants to share it with others.
“Close your doors. Sleep with your doors closed. Doors being closed will allow you additional time and it’s the biggest way to reduce fire spread."
Loudon County Emergency Communications Center said multiple fire and rescue crews from the surrounding area arrived at the scene to put out the fire, which Janae said engulfed their home within seconds after they got out. Loudoun County ECC said the cause of the fire was a combustible placed too close to a heat source. Damage estimates are nearly $700,000.
“It’s not until you encounter fire and see how fast it spreads that you really have to take action to protect yourself," said Janae.
Now loved ones of the Wheeler family are asking for the community’s help to give to a family who has a track record of giving to others. Janae and Sir have been running their business VVS events together for 12 years. Helping to support non-profits and bringing communities together with fun events. A friend of theirs started a GoFundMe to help the Wheelers get back on their feet. And Janae said she is so grateful for the help they’ve already received like coats and meals.
“In a record amount of time, the kids have what they need to get back to school and the love of our friends has just been a surplus of comfort to help us see the light."
She said continuing with their events has also helped.
“It’s really has been a grounding source for us right now to focus on the positive and keep moving forward," said Janae.
